Step 1: The default incident ticket form is as follows: If you wish to add new sections, please follow the next steps.
Step 2: Go to Settings > Helpdesk > Custom Form and you can use available fields for the custom form.
Step 3: You can utilize the Category, Urgency, and Impact fields in the incident form customization section as shown above. Once you use them, you will be able to see changes on the incident form from the user portal.
